In person at the Financial Aid Office (Located in the North Atrium on the first floor of the Ben Parker Student Center behind the Cashier’s Office)

- Parent Income Adjustment Forms:
  - Estimated Income Adjustment: You can also use this form if you are unsure which tax year would be most beneficial for you. We will run a simulation on the FAFSA to determine if an official change is warranted.
  - 2023 Income Adjustment: This form requires you have already filed your 2023 taxes and include documentation with submission.
  - 2024 Income Adjustment: This form is accepted beginning May 1, 2024.
- Student Income Adjustment Forms:

Student Award: This form allows departments to provide financial awards/prizes to students from their index. Student Award Forms can be submitted through OnBase (Onbase > Open Workflow > AR3-Student Award Workflow).

Satisfactory Academic Progress Appeal Form

Students who do not meet the minimum standards to maintain their scholarship or financial aid eligibility are able to appeal. If your scholarship or financial aid has been terminated, you can review the Satisfactory Academic Progress standards here. To appeal, submit this form with a personal statement and any supporting documentation you may have.

Scholarship Forms

Outside Scholarship Donor Form: Scholarship organizations can use this form to submit with their scholarship payments to the Mines Financial Aid Office. Using this form is not required.
Outside Scholarship Donor Release of Information
Outside Scholarship Notification. Please use this form to notify our office of any scholarships you have been awarded by a private donor.
Student Award: This form allows departments to provide financial awards/prizes to students from department accounts. Student Award Forms can be submitted through OnBase (Onbase > Open Workflow > AR3-Student Award Workflow). To award from a scholarship fund, please email scholarship@mines.edu.
