Submit forms to the Financial Aid Office

Forms will not be accepted without student CWID and name
  • Email to finaid@mines.edu  or upload securely to Trailhead (preferred).  
  • In person at the Financial Aid Office (Located in the North Atrium on the first floor of the Ben Parker Student Center behind the Cashier’s Office)
    Monday through Friday, 8 am–5 pm
  • Mail: Mines Financial Aid, Colorado School of Mines, 1200 16th St., Golden, CO 80401.

Contribution Review/Professional Judgment Forms

Submit one of these forms to request a financial aid review based on family financial circumstances not reflected on the FAFSA. Reviews begin March 1 and end February 28 for a specific academic year.  We will begin taking COVID-19 based adjustments on June 1 to try to capture the fullest effect of the reduction.

Satisfactory Academic Progress Appeal Form

Students who do not meet the minimum standards to maintain their scholarship or financial aid eligibility are able to appeal.  If your scholarship or financial aid has been terminated, you can review the Satisfactory Academic Progress standards here.  To appeal, submit this form with a personal statement and any supporting documentation you may have.

Scholarship Forms
  • Outside Scholarship Notification. Please use this form to notify our office of any scholarships you have been awarded by a private donor.
  • Barnes and Noble Textbook Scholarship Application. Check back in November for spring 2021.
  • Student Award. For department use to award a student a prize or other award.
