Submit forms to the Financial Aid Office

Forms will not be accepted without student CWID and name
  • Upload securely to Trailhead (preferred).
    • Current Students can upload through Trailhead>Financial Aid card>Upload Requirements
    • Incoming Students can upload through Trailhead>Incoming Undergraduate (or Graduate) card>Financial Aid>Requirements>Upload Documents
  • In person at the Financial Aid Office (Located in the North Atrium on the first floor of the Ben Parker Student Center behind the Cashier’s Office)
    Monday through Friday, 9 am–4 pm

Scholarship Forms
  • Outside Scholarship Donor Form: Scholarship organizations can use this form to submit with their scholarship payments to the Mines Financial Aid Office.  Using this form is not required.
  • Outside Scholarship Donor Release of Information
  • Outside Scholarship Notification. Please use this form to notify our office of any scholarships you have been awarded by a private donor.
  • Student Award: This form allows departments to provide financial awards/prizes to students from department accounts. Student Award Forms can be submitted through OnBase (Onbase > Open Workflow > AR3-Student Award Workflow). To award from a scholarship fund, please email scholarship@mines.edu.
