Institutional Verification Form (IVF)

This link is for the Institutional Verification Form. If your "Tracking Items" screen on Web for Students shows that this document has been waived or received, you do not need to complete this form.

The pdf document can be completed on your computer but

  • must be printed out

  • signed by the student

  • signed by parent/guardian and returned to the Financial Aid Office with any other requested documentation

    Completed verification forms should be submitted to the Financial Aid Office - Room 061 in the Ben Parker Student Center during regular business hours: Monday through Friday, 8 am to 5 pm.  The forms may be faxed to 303.384.2252
